Second and last, does anyone have any idea why TLG has never ( as far as I
know ) introduced such a part.  My own idea is that it may deal with the
moulding process and the amount of resin in such parts.

Try the 1 x 1 Plate with Headlight.  There was a seriously long and
informative thread on this earlier, under the title "Gender Changers" or some
such.

(...) Is there a way (or rules) to prevent non-TLG parts from creeping into the official LDraw updates? That seems like the only practical/logical danger...any other objections (like mine) would be personal/subjective. I don't use LDraw/LEdit enough (...) (25 years ago, 10-Feb-99, to lugnet.cad.dev)
