Ok, let me clear this one up for you. In one of Tim Zahn's books, Luke
finds out(from Mara Jade) that Vader's hand was cut off by Palpatine in
punishment for his failure at the Death Star. That would have been before
SOTME. In SOTME Vader's arm is cut off by Luke at the elbow. Luke was
"suprised at how little he(Vader)bled. Why did he bleed so little? Becease
his hand was fake! The one Luke cuts of in ROTJ is a replacement for the
one Vader lost in SOTME, which was a replacement for his real hand, which
was cut of by the Emperor. Got that?

Oh, and here's
something that makes SOTME fit well with TPM: in SOTME Vader turns R2-D2 and
C-3PO off. When the 'droids are turned on, threepio says that they tried to
resist, but Vader knew all the access codes he needed to shut them down.
Now we know how he knew.
