But this kind of thing isn't done for other parts, is it? Take the
bushes, wheels, gear cogs and so on; they must all be rotated to fit the
axles.

We could just argue that the cam is essentially a pulley/wheel kind of
part, rather than a liftarm. In that case, it is already rotated
properly.

I feel that the cam is closely related to the parts refered to as
"liftarms" in LDraw, which includes the "halfbeams". And they are all
flat.
Of course, the rotation of a single part does not matter too much, but
we recently had a discussion about the importance of a consistent
default rotation of LDraw parts.
