1) rare discontinued items (...)
2) rare items where the molds still exist (...)
3) promotional items: the gold and platinum bionicle masks, etc.
4) prototype items or items never officially released (...)

Items that go under (4) are probably the rarest of all as such items mostly
accidentally leave the facturies, only in very small quantities. Speaking here
of more elaborated parts like trees or minifigs etc., not the regular 2x4 brick
of which lots of test-elements in various colours are quite commonly available.
As for (3): There is a nice little element that a friend of mine happens to own
(pic by TF):
Another rare item that goes under (3) (as it is promotional) is the printed(!)

As for (1) and (2): A part thats only used in one set that was not broadly
available all over the world is - of course - rarer than unique parts in common
sets that were released in large quantities. So this infamous 2x10 brick in
trans-clear is without any doubt much rarer than a banal 2x2 tile in trans clear
from 6590, although this clear tile is *considered* rare because it is at least
considered at all while the existence of this 2x10 brick is not even well-known.
