I've had a few, but most of them have been in the very early US availability of
the sets. I bought one of the first two UCS X-Wings in town, and it was missing
a 1x1 round plate (I ended up substituting with the included spare piece in the
other grey color). My first two or three TECHNIC R2-D2 sets (still, IMHO, the
all-around best SW set) had 2x #5 and 1x #6 white angle connectors when they
should have had 2x #6 and 1x #5. The only time I've had an error show up well
after the set became available was a Turaga Vakama set that was missing his
firestaff.
I remember hearing someplace that they weigh the inner baggies and/or boxes
before shipping them, but the weight has to be programmed for the correct
assortment, and the sorting machines have to be programmed to pack the correct
parts. Once everything is programmed correctly, they've probably got a really
good track record, but Consumer Affairs will take care of you when you do get a
bad pack. Granted, getting the right piece is still always going to reflect
better on them than supplying the correct replacement part.
