Depends on how you ship...
I live in the Netherlands, and recently sold some stuff to people in the
United States, Norway and Japan. From the dutch postal service flyer:

To US:

- under 1 kg : $15 for priority ($17 insured) and $12 standard. Economy is
$8 I think
- 1 to 5 kg  : $24 for priority ($26 insured) and $18 standard. Economy
about $13

If value is above $500 for every $500 add $2 extra for insurance. That's
roughly it, Japan is about 20% more expensive, Europe is about 20% cheaper.
I might be off something like $1 because of exchange rates though

And payment... I usually use Larry's Factoring, wire the money or send cash
(sign on delivery, so I always have proof of payment, and shipment is
insured. I learned that one the hard way :-( ) Wiring the money costs me
about $3 + exhange rate losses.
Cashing US cheques is about $5 to $7 for me, and I've never even tried
money orders, I guess those are about the same. When I sell something on
eBay I usually "push" users to send cash or use factoring. This way I can
keep costs down. Now if only X or Paypal would become available in the
Netherlands as well :-(
