If you lost your password, follow these simple steps:
1. Notify the Commander-in-charge immediately of the category 5 security
breach (you did that).
2. Fill out forms MIL-5111STDPW-Q (password selection form),
LUG-2946693-V (membership validation form), C99-TVSHKE (certifiaction
of lost password status), PP-2569433-Z (pretty please voucher).
3. Note that your new password selection must conform to MIL-629
and Purple Book Class 6 standards, or a conforming password will be
selected for you.
4. Return copies 5,9,12 and 14 of all forms except MIL-5111STDPW-Q, which
you should return in it's entirety. Keep all other copies for your records.
5. Please wait 12-14 months (24-36 months if Password request accompanies form
AZ-29345-W auczilla winnings request).
