I have fixed what I hope is all of the 'broken thumbnail' problems with
the gallery.

New uploads should not result in broken thumbnails under any circumstances.
Broken file thumbnails can be fixed by issuing the 'reload' command from within
the folder containing the broken images.

Broken _folder_ icons can be fixed by issuing the 'reload' command from
*within* the folder with the broken icon (not it's parent where you see the
broken image).

Here's how it works now:

File thumbnails are created based upon the file extension.  If the file has a
correct extension, and the gallery knows how to generate a thumbnail for that
file type, the thumbnail will be created.  If the extension is unknown, *or*
incorrect (a JPEG with a .gif extension or a GIF with a .jpg extension for
example), then the default text icon will be set as the thumbnail.  If a file
has a correct extension but the thumbnail generator still creates a bad
thumbnail (as with some .bmp files), the default text icon will be set as the
thumbnail.

Files with spaces in the filename are currently not supported and are
automatically deleted.  Files with certain extensions (like .html .htm and
.mp3) are also deleted.  Deleted files will not show up at all in the gallery.

Zero length files (sometimes caused by aborted uploads) are deleted and will
not show up at all.

If you keep uploading a file, and it doesn't show up, make sure it's not a zero
length file (on your computer), and try changing the file name to something
reasonable.  Try using just regular letters, numbers and a proper extension.
If it shows up as the default text icon (or some other generic icon used in the
future), make sure the extension properly reflects the format of the file.

folder thumbnails are set to the default folder icon when the folder is
created.  When files are uploaded, deleted or the reload command is used the
system sets the folder thumbnail to the first file thumbnail in that folder.
If there are no files in that folder, the folder thumbnail is reset to the
default folder icon.  In the past, the folder icon would still be set to the
non-existant first file, resulting in a broken thumbnail.

Other changes for 0.10.4:
- ALT tags now reflect the target content (folder, file or member name)

There are many other features, enhancements and bug fixes on the way, but this
one was huge, annoying and had to be fixed asap.
