The sync rates are determined by the electronics inside the monitor.  More
expensive monitors can do higher sync (and hence higher refresh) rates.

(...) Really it's a combination of the video card and the monitor. The video card has to be able to generate the high sink rates, and the monitor needs to be able to display them.
