I found a comment card exactly like this one in an MTT I picked up last week,
and it asks for customer input in a generic way. It was the typical "how many
sets do you own" and "what themes do you like" sort of questions.
One thing I found very interesting was some of the commentary could be used to
figure out how many AFOLs are returning cards like these. It had a birthday
question, and then it asks specifically if you bought it for yourself or as a
gift. It also had a section at the bottom where you could specifically
request that it would be okay for Lego to contact you and an address/phone
block.
