What do other people do? Surely you don't all use the same password for
everything? My manual scheme (which I am not going to describe but it does
result in different passwords) is breaking down, too many sites.

I run KeePass off of a thumb drive:

Its an encrypted database app that will store user ids, passwords, urls, notes,
attachments, etc.  It also features a group/category feature which allows you to
organize everything.  A rule based pwd generator is built in.

I run app from a batch file (so drive letter doesn't matter) which also copies
the db file to the c: drive in case I lose the USB drive, I've got back-ups of
the db in a few places.
