Which basic issue is he circumventing?  I thought the basic issue was
determining who should pay for Random Joe's medical care.  Larry thinks
Random Joe should and Thomas thinks that Larry should.

medical service irregardless of the ability of the patient to pay for that

Hippocrates developed the oath, but I don't recall it saying anything
about working for free.  Mostly it's this kind of Illuminattiesque
treatment of who gets to learn medicine and it throws in a thing saying

Why don't you communists ever see that the tragedy of the commons is a
real and unavoidable thing?
