Why the difference?
Probably due to the fact that if it gets stuck in someone's throat
there's a good chance they'll still be able to breathe (like pen-lids) - • It
also makes them easier to remove (Getting the non-hole heads of my • classic
spacemen is really hard - and if the helmet's stuck on as well... - • although
pliers with some sort of rubber padding could work - anyone tried it?)

I have no idea when, it occured during my dark ages (1986 - 1995).

The "WHEN" was in the early 1990's.

Indeed. My guess is 93-94, perhaps? I thought they might have done it to
reduce the plastic in each head, for a cost saving measure. It might be a
small amount on a few, but when you produce several thousands,  you are
talking real numbers.
