Why the difference?
    Probably due to the fact that if it gets stuck in someone's throat
there's a good chance they'll still be able to breathe (like pen-lids) - It
also makes them easier to remove (Getting the non-hole heads of my classic
spacemen is really hard - and if the helmet's stuck on as well... - although
pliers with some sort of rubber padding could work - anyone tried it?)

I have no idea when, it occured during my dark ages (1986 - 1995).
