I realize we can do some of this with PF, albeit limited.  Has anyone used any
of the MRC or Digitrax model railroad systems (DCC and sound) successfully with
LEGO?

My pipe dream is to have sounds, lights, etc per each locomotive.

We have some of that here in Edmonton, Canada. I think 3 of our members have DCC
rigs, and more have DCC-modified trains. The mainlines on our train layouts have
been DCC for several years - we use the old 9V analog stuff for independent loops.

This year (web page not up yet) our DCC head honcho had a large Canadian Pacific
dual-powered engine with all DCC. It has lights at both ends, run by the DCC
decoder chip (so only the one in the direction the engine is moving lights up).
The DCC decoder he used also has sound features - that works by having a small
speaker connected to the decoder chip. Unfortunately, the brick body of the engine
(it is a large 8-wide) muffles the sound quite a bit, so it is hard to hear.

Some of our members have several DCC locomotives, and run multiple on a train
(Kevin's DCC controller was getting hot at one point, even though he has a DCC
booster with it!). However, I don't think anyone has been brave enough to try
out a DCC "consist" yet, where you can tell the controller to run multiple
engines together. We haven't done tuning of the DCC control parameters much, like
the acceleration and deceleration rates.
