If you are talking about plain Lego (which rules out wrapping things up in a
perspex case with rounded edges and deburred, bevelled holes for ears, and eyes
set behind clear area), then my first recommendation is using studless
techniques to build a cage around the real vitals.
If you want an aesthetic nose, clearly avoid anything attached with studs only,
and probably recess the attached zone behind the cage, with a section protuding
out from it.
Since you can get some fairly nice angles from the studless beams, you are not
limited to it being a box. Eyes can be happily recessed into the cage, although
you will want to consider putting some Lego windows (with their studded
atatchmetns behind the caged area) in front of them so little fingers cant pry
off the elements.
Get yourself some of the recommended rubberised sheets - you are in for a lot of
fiddling wiith pins, and it would be quite a pain to pull apart, but it would
also be pretty resistant to toddler attacks.
