My guess is either they ran out of inserts or yours fell out of the catalog or
the person places the inserts in the proper place to get stapled in was out to
lunch. Anyway, the catalog number is #SP01 ($11.96 w/o order-- why would you
use that?) or #SP02 ($7.99 with order). My catalog code (I had an insert) is
J111. HTH.
BTW mine from a couple of weeks ago contained exactly what Kevin's contained.
