I'm still looking for a source for full-page laser printer form that has a small area with peel off label. Not a page size sticky label (these cost 42 cents and still have to be cut up.) (...) You may want to try Uline. They have the various label types that start at $30 for a box of 100 sheets. That gets you down to ~30-cents page even if they are not exactly what you seek.
