Just started using LPub 4 (last used LPub 2!) and came across this. What seems
to happen is that the callout placement meta-command *is* created, just after
the callout end meta-command. So after moving the callout with the mouse you
need to cut-and-paste the meta-command in the text window inside the callout
begin/end section and then refresh the page.
