I actually got the idea for the headrests here:

If I hadn't seen that, I probably would have just used a 1x1, 1x2 tile, or
1x1 tile with clip.

I think thinking outside the brick, as you call it, comes from building for
a long time and looking at what other people have done. For example, I've
been building with LEGO since I was 4, so almost 15 years now with very few
interuptions. And I've been looking at LEGO models on the Internet for 6
years. And even then, I still build primarily studs up, with only details
going in other directions.
