There have always been Infected Haus in mask packs, they are just a little rarer
than most others. So that wasn't a fluke or mistake and Lego Direct had nothing
to do with it.
In lugnet.technic.bionicle, Drew Dirschell writes:

Bionicle Masks box from Lego Direct.  I thought one could only get the
infected maskes in set 8538, Muaka and Kane-Ra.  Another point of interest
is that the mask in the the box with the infected Hau was a blue Hau.

Hopefully this means Lego Direct is throwing some of the rare masks in the
mix at random, but maybe it was just a mistake.

I'm visiting LegoLAnd California next weekend for my first time, ¿Do they
still have the transparent green Miru masks?  I hope so.  Lewa is my
favorite, and I'd like to get a complete set for him.

This may not be news to anyone else, but I got an infected Hau in the 8525 Bionicle Masks box from Lego Direct. I thought one could only get the infected maskes in set 8538, Muaka and Kane-Ra. Another point of interest is that the mask in the the (...) (23 years ago, 20-Apr-02, to lugnet.technic.bionicle)
