Section 1 - Who are you (Please type your name between the following brackets)?

[ ]

Section 2 - Your votes

Please vote for one entry in each category by marking an ‘X’ in between the
square brackets of your choice if you are the
only one to enter a particular catagory please mark the entry with an ‘N’ in
between the square brackets.  In categories where you are competing in a
category against other opponents -- YOU MUST VOTE AGAINST YOURSELF BY VOTING
FOR SOMEONE ELSE!.  All contestants are expected to vote once in each
category.  Failure to vote at all means that you are disqualified (through
your own inaction). Failure to vote against yourself in key categories
(::wink, wink -- they won't notice::) means that you are disqualified (You
must be big enough of spirit to vote for others).

17 ballots are expected -- we expect 100% voting participation.
